New Incoming Student-Athlete Information

Hello,

As a New Incoming Stonehill College varsity athlete, we’d like to welcome you to the Stonehill College Athletic Family! As an incoming new student-athlete, there are specific sports medicine documentation requirements you must complete prior to being allowed to participate in any practices/games/conditioning sessions with your respective team(s).  In this email and attached above you will find all the required information/instructions you will need to complete your athletic medical paperwork requirements. PLEASE READ ALL INFORMATION CAREFULLY!

Attached above as well is the Stonehill Secondary Insurance Policy Information.  Please take time to read through and fully understand this very important information Make sure your parents and/or guardian gets/sees this information.  It will help guide decisions on health insurance policy that is best for you as a student-athlete.

You will find all your medical required paperwork in your ARMS account.  You will be receiving an email from ARMS and /or your coach on how to access your ARMS account soon.  As part of your requirements, you will also be asked to create an ATS medical account. This account will serve as your athletic medical file for the time that you are competing at Stonehill.  You will be asked to upload your primary Insurance information and picture of front and back of Insurance card into your ATS account (See above attachment for ATS instructions).

Below is a check list of your required items to complete with reference to above attachments containing important information and instructions.  These items are for all new incoming student-athletes (freshman, transfers, and Grad-students). 

  • Register for an MRN# (medical registration number) with Mass General Brigham’s by calling 1-866-489-4056.  Please have your primary health insurance information ready. Enter given MRN# into ATS account under the General tab in the Alternate ID section.
  • ( CLICK HERE ) Create your ATS medical account following the New Student-Athlete ATS Instructions
  • ( CLICK HERE ) Upload all 4 pages of current (within 6 months of playing season per NCAA) physical into ARMS using the blank PHYSICAL FORM **PLEASE NOTE: This athletic physical is separate from the medical health form you submit to health services as a student. You must do both. **
  • ( CLICK HERE ) Upload a copy of a Sickle Cell Solubility Test Results in ARMS (found also in Form A in ARMS)
  • ( CLICK HERE ) Complete a Baseline ImPACT Concussion Test with Stonehill’s Customer ID.
  • **Only for Student-Athletes that are Diagnosed with ADD/ADHD and taking prescribed medication**
    • ( CLICK HERE ) Have Physician fill out the ADD/ADHD Medical Exception Form and Upload it into your ATS account under the E-Files Tab.

The Following Items are all accessed through ARMS

  • Fill out Form A (Additional Information Acknowledgement Form) found in ARMS
  • Fill out Form B in (Assumption of Risk Form) found in ARMS
  • Fill out Form C (Athlete Concussion History Baseline Assessment Form) found in ARMS
  • Fill out Form D (Concussion Acknowledgement Form) found in ARMS
  • Fill out Form E (Consent for Treatment Form) found in ARMS
  • Fill out Form F (Health History Questionnaire Form) found in ARMS
  • Fill out Form G (HIPPA Form) found in ARMS
  • Fill out Form H (Medical Release Form) found in ARMS
  • Fill out Form I (Prescription Medication and Supplement Notification Form) found in ARMS

The Deadline for all the above requirements:

Summer Access Athletes (MB, WB, & FB) – Prior to Arriving to Campus

All other New Student-Athletes – July 15th

These requirements and information are all necessary for the sports medicine team to help provide the best possible athletic health care to you during your athletic career here at Stonehill College.  I ask that you thoroughly and carefully go through this information and follow all instructions.  Please fill in ALL the information into ATS accurately and fully!  If you or your parents have any questions or concerns about any of the information found here and/or the ATS system, please feel free to contact me.


Some FAQs:

  • The NCAA requires that I need to submit a physical within 6 months of my participation, but my insurance only covers my physical on a yearly bases.  My season starts in August and my last physical was in December of 2017, will that count?

The NCAA states physical should be within 6 months of start of playing season. However, knowing insurance only cover physicals on a calendar year, we ask that you submit you most current physical within the last 12 months. Then upload your new physical as soon as you get it. Please communicate with sports medicine staff if your most recent physical is beyond the 6 months requirement.

  • Do I have to submit a sickle cell test documentation? I read that the NCAA allows you to waive out of it.

Yes, you must submit a sickle cell test result as a new student-athlete.  Stonehill Sports Medicine does make it required.  You should be able to obtain a prior test result done at birth from your pediatrician or hospital.  And if you cannot locate a prior test, then you can have your primary care physician order a new test or as a last option, pay out of pocket through an online prescription service.  See above attached sickle cell information.

  • If I turned in my medical form/physical for health services during orientation, do I still need to submit a physical to athletics?

YES!!  The physical form for health services DOES NOT satisfy the athletic physical form requirement.  Also – PLEASE DO NOT HAND ANY OF THE ATHLETIC MEDICAL INFORMATION IN AT ORIENTATION

  • I’m trying to upload stuff into ATS like my insurance card photo but it won’t upload?

There is a 1MB limit for all uploads into ATS.  When you scan and/or save a photo of card or documentation onto your computer, save it as a medium file.  Or go into edit feature of photo and downsize the pixels to 50% and then resave.

  • If I did a baseline ImPACT concussion test in High School, do I still need to take a new baseline test?

YES!  As an incoming new student-athlete for Stonehill College you need to have a Baseline Test under Stonehill’s ImPACT account. We cannot use your high school ImPACT test scores.
